Marching Band Physical Form
Every student in band is required to have a physical in order to participate. If a student is also involved in campus sponsored sports that also require a physical, then only one form is required to be filled out for the school. Physicals are only valid for one school year and will need to be completed annually for each school year in order to maintain UIL eligibility and compliance. These are due by August 1st.

Once you have a "Cleared" physical form from your doctor, your parent or guardian must go to rankonesport.com and fill out the coordinating forms for students attending Memorial High School.
